Position Summary
The Medical Phone Triage Coordinator provides general indirect nursing care through patient phone triage, education, care coordination, and clinical follow-up. This role requires excellent clinical judgment, strong problem-solving ability, and outstanding interpersonal skills, all while maintaining compliance with Noble policies and departmental protocols.
Key Responsibilities
- Assess patient phone calls and provide appropriate intervention per guidelines, standards of care, and state/federal regulations.
- Perform RN triage duties, including patient education, addressing abnormal lab results, transitional care management, and resolving phone messages outside the CMA/MA scope.
- Accurately document all patient interactions in the medical record.
- Manage tracking systems to ensure compliance with policies, medical guidelines, and health maintenance standards.
- Participate in multidisciplinary teams to improve patient care processes and outcomes.
- Provide excellent customer service while diffusing potentially challenging situations.
- Maintain patient confidentiality in all interactions.
- Manage, track, and complete ER follow-ups as needed.
- Follow policies to resolve patient complaints and concerns.
- Demonstrate regular and reliable attendance.
- Adhere to Noble’s code of conduct, policies, and procedures.
- Perform additional duties as assigned.
Qualifications
- Current State of Wisconsin Registered Nurse License or Nurse Compact License required.
- Outpatient clinic experience, preferably in Family Medicine.
- 3-5 years of nursing experience preferred.
- Supervisory experience of an interdisciplinary team preferred.
- Knowledge of implementation and maintenance of QI program preferred.
- Strong knowledge and ability to use MS Office and EPIC.
- Basic Life Support (BLS) Certification required.
