Registered Nurse (RN) Case Manager
Full time- 40 hours per week
Shift - Monday through Friday
Location: Adena Pike Hospital, Waverly Ohio 45690
Position Summary
The RN Case Manager is a key member of our interdisciplinary healthcare team, collaborating across the continuum of care to advocate for patients, guide care planning, and coordinate resources. This role ensures patients, families, and caregivers are informed, supported, and empowered to participate actively in their healthcare journey—aligning every action with the mission, vision, and values of Adena Health.
The RN Case Manager combines clinical expertise, critical thinking, and compassion to ensure that each patient receives the right care, in the right setting, at the right time. This position supports optimal health outcomes, seamless care transitions, and the responsible use of healthcare resources, all while complying with state, federal, and payer-specific regulations.
Qualifications
Required Education: Bachelor’s Degree in Nursing (BSN).
Major/Area of Concentration: Nursing.
Five (5) years recent clinical experience or 3 years of inpatient hospital
experience
Required Certifications, Credentials, and Licenses:
Registered Nurse (RN) with a current Ohio license.
Preferred Certifications:
Certified Case Manager (CCM®).
Acute care and/or case management experience preferred.
Strong communication, multitasking, and decision-making skills.
Demonstrated compassion, empathy, and adaptability in fast-paced environments.
Knowledge of utilization review, discharge planning, and care coordination best practices.
