LPN JOB DESCRIPTION
Job Summary: The role of the LPN is to support the RN to ensure that the health and safety of all tenants are met. The LPN will assist the RN to ensure that staff is trained in medical procedures and policies which are current and up to date. Must be able to relate professionally with tenants, staff and family members, display a positive work attitude and be able to work as a team with other staff members.
Job Relationships:
Reports to: Home Health Director
Interrelationships with: Registered Nurse, Sunrise Village Manager, Sunrise Village Assisted, Living Coordinator, Park View Village Manager, Sunrise Village Housing Counselor, Personnel Attendants, Housekeepers, Dietary Staff, Maintenance
External Contacts: Visitors, Families, Physicians, Pharmacies, Discharge, Planners, etc
Hours of Work: 30 Hours per week. Rotating schedule between Sunrise Village and Parkview Village
RESPONSIBILITIES
- Participate in implementation of service agreements with new tenants as requested.
- Monitors and assists to maintain tenant’s charts and clinical progress notes.
- Communicate to physicians and other personnel changes in tenant's needs or conditions.
- Facilitate the coordination of tenant services with other service providers.
- May be responsible for ordering medications and medication renewals as well as any necessary follow-up with physicians regarding medications and/or medical conditions.
- Assure that all physician orders are signed and implemented with-in 24 hours.
- Complete medication set-ups where appropriate.
- Assure that medication administration procedures are followed properly and discuss any needed corrections, errors, etc. with staff.
- Review new medication sheets each month prior to placement in tenant apartments. Initial and date.
- For each tenant receiving help with medication administration, review medication sheets monthly and assure accuracy and appropriate signatures.
- Communicate in tenant's chart any information that staff needs to be aware of.
- Perform every other supervisory visit.
- Work as a team player with all members of the care team.
- Provide health and medication training to new employees as well as continued education for all staff and document such training to assure compliance with state and other regulations.
- Supervise medication practices and medical equipment being used in the building.
- Assure that staff clearly understands and follows all tenant care procedures.
- Shares tenant and family concerns with the appropriate staff.
- Assist in the development of policies and procedures and implementation of them.
- Attend staff meeting, as requested.
- Work with building manager to assure emergency procedures are in place.
- Is flexible and willing to expand and adapt to changes in the AL industry.
- Assure compliance with regulations and is available for state licensure surveys.
Certifications, Licenses, Registrations:
- Must be registered and licensed with the State of Minnesota as a LPN and must maintain licensure.
- Must have a valid driver's license.
Working Conditions:
Exposed to body fluids, infection, odors and behavior of tenants.
Physical Demands of the Job:
- Must be able to move throughout the workday and throughout the building.
- Must have adequate vision and hearing to meet the requirements of the position.
- Must be able to cope with the mental and emotional stress of the position.
Mental Demands of the Job:
- Must be able to act independently and use independent judgement.
- Must make decisions as the need arises on tenant status.
