Per Diem Advanced Practice Provider – Acute Care Consult Service (Physical Medicine & Rehabilitation) :

 

We are seeking a compassionate and skilled Per Diem Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Acute Care Consult Service within the Physical Medicine & Rehabilitation (PM&R) department. The APP will provide consultative services to hospitalized patients requiring rehabilitation-focused care and support functional recovery across a variety of medical and surgical services.

In this role, the APP will work collaboratively with PM&R physicians, nursing staff, and interdisciplinary teams to assess patients, develop rehabilitation care plans, and facilitate appropriate discharge planning and therapy recommendations. The position requires strong clinical assessment skills, excellent communication, and the ability to function independently in a fast-paced, consult-based environment.

 

Responsibilities:

·        Perform inpatient consultations for PM&R across acute care settings.

·        Conduct comprehensive functional assessments and recommend appropriate therapy services (PT, OT, SLP).

·        Collaborate with attending physiatrists to create and implement rehabilitation care plans.

·        Facilitate coordination of care with interdisciplinary teams, including case management and therapy services.

·        Document findings and recommendations clearly in the medical record.

·        Participate in quality improvement and patient safety initiatives as needed.

Reports to: System Chief of Rehabilitation

 

 

 


        Licensed as Advanced Practice Nurse Practitioner (APRN) or Physician Assistant

        Minimum Five (5) years of clinical experience

        AP Team Leadership experience preferred
