What You’ll Do
As an Outreach Worker in the FIT program, you will provide high-quality, community-based services to youth with Serious Emotional Disturbance (SED) and their families. You will work as part of a multidisciplinary team to deliver intensive treatment, support, and skill building to help stabilize youth in their home and community settings.
- Deliver intensive in-home and community-based services 3–5 times per week, as clinically indicated
- Teach and coach youth in coping skills, self-care, and strategies for managing emotions
- Support families in addressing the emotional and mental health needs of youth
- Collaborate in the development and implementation of individualized treatment plans
- Provide behavioral coaching to enhance problem-solving, communication, and family dynamics
- Facilitate linkages to community resources and make referrals through warm hand-offs
- Co-facilitate team meetings and assist with aftercare planning
Who You Are
- Associate’s or Bachelor’s degree in Human Services with relevant experience, or enrolled in a Master’s degree program
- Experience providing therapeutic support to youth and families preferred
- Knowledge of mental illness diagnoses, behavioral modification techniques, and coping strategies
- Familiarity with child-serving systems and community resources
- Strong communication, time management, and organizational skills
- Ability to work with diverse populations and demonstrate cultural competence
- Valid driver’s license and reliable transportation required
