Under general supervision, this position will perform differential response and case management services to clients in accordance with the funding contract.  This may include, but is not limited to needs assessments, referrals to community resources, one-on-one coaching, CFT coordination/participation, follow-up contact, up to 25 home visits per week with a client caseload of up to 40 case management clients a month, weekly reports, documentation, data collection, file maintenance, coaching, mentoring, and social service support to the client.

Position Summary:

Under the supervision of the Parent Coaching Program Manager, this position functions as a direct link between children and families in accordance with the designated funding contract.  This position provides intensive Family Strengthening Services, to CAL WORKS families, which can include but not be limited to any and/or all of the following: Home Visitation Services following the Parents as Teachers (PAT), and Raising A Reader (RAR) curriculums, Comprehensive assessment of children and families using the ASQ and Protective Factors tools, Referrals to community resources, and Facilitation of Parent Workshops.  Additionally, this position is responsible for outreach, intake and enrollment of children and families into the program. This position will conduct home visits to the families enrolled in the program and will build/maintain a caseload of 17 families at any given time.

Program Specific Duties:

1.    Maintain family case files on a weekly basis and assist with any related statistical and/or evaluation components.

2.    Maintain all tracking spreadsheets on a weekly basis and assist with any related statistical and/or evaluation components.

3.    Provide intense home visitation to assigned families maintaining fidelity of designated curriculums when applicable.

4.    Attend meetings as required by CAPC and the contract.

5.    Provide concrete activities and education to strengthen parent and child attachment and bring the child into focus.

6.    Teach problem solving skills to enhance family functioning and assist in setting and achieving family goals.

7.    Support families with job search and job readiness assistance.

8.    Support families in providing a healthy and safe environment for children.

9.    Coach parents to act as advocates for themselves and their children.

10.    Provide developmental and behavioral assessments of children.

11.    Establish trust with families through family centered support and encouragement.

12.    Have a solid working knowledge of all the rules, regulations, and laws that apply to the contracts.

13.    Promote and maintain a sense of teamwork between assigned department and other CAPC staff, First 5, community partner agencies, and other individuals relevant to the successful case management of each case.

14.    Network with other community agencies and participate when requested, in community and health fairs and other events designed to promote CAPC programs.


Minimum Qualifications 

 

Education: Graduation from an accredited four year college or university with major in social services, child development or a closely related field, preferred. 

 

Experience: Two years of relevant experience working with children and families. Experience and enthusiasm for working with and supporting families; especially parents/caregivers.


Knowledge of:  Childhood development, and child abuse issues. Knowledge of community-based and social networks and resources.  Strong knowledge of and a demonstrated ability to work with a culturally diverse population.
