As a Care Access Coordinator, you’ll play a vital role in connecting health plan members with life-changing healthcare services. You’ll be the engine that keeps our Care Access team running smoothly, ensuring appointments are back on track and that our Care Access Pros are where they need to be, when they need to be there.
In this role, you’ll take the lead on rescheduling appointments for previously scheduled members, helping ensure they receive the care they need. You’ll also be the go-to expert in coordinating seamless travel, from booking flights and accommodations to arranging transportation, all while managing travel costs and troubleshooting any unexpected issues.

Primary Responsibilities
Run daily report to move member appointments on calendar as requested by CAPs
Respond to R/S Updates Channel to update member appointments as requested by CAPs
Coordinate reschedule campaigns based on CAP, City, and active or upcoming rosters
Work with CAP Managers to determine new scheduling dates and CAP assignments for reschedule campaigns
Assist CAPs with reschedule outreach as needed in case of CAP emergencies, out sick, etc.
Place large volume of outbound calls to schedule appointments
Respond to member eligibility channel in Slack and add member appointments to CAP calendar accordingly
Monitor and track rescheduled members on a daily and weekly basis via Salesforce reporting
Place follow up calls regarding member screenings as directed by management
Respond to inbound calls/voicemails
Troubleshoot, problem-solve and escalate member issues to management as needed
Arrange all roster-related CAP travel including flights, hotels and rental cars
Monitor and respond to the Travel Requests Slack channel
Coordinate receipt of itineraries to CAPs and CAP Managers
Maintain an organized and up to date tracker of all requested travel and related costs
Maintain CAP Rental Car tracker to ensure all rentals are returned in a timely manner
Resolve and assist with travel escalations
Ensure that all travel arrangements are made in a timely manner
Ensure all requested and booked travel adheres to the HealPros travel policy guidelines
