Responsible for the provision of family case management services to the families in the Amarillo area and surrounding communities, to include skills building, goal setting and tracking, maintenance of files and related activities. Serves as liaison between families and other support agencies.
Essential Duties:
- Fosters healthy communication between clients and their families to promote the goal of improved relationships, documents such contact, and communicates the results of these family contacts to supervisor.
- Refers clients and families to any additional resources deemed helpful in accomplishing family/client goals.
- Maintains case files that are current, accurate, confidential and in compliance with all applicable standards.
- Works with clients and their family to develop goals related to achieving a healthy home environment.
- Provides monthly or bi-monthly in-home visit with each family receiving services.
- Provides follow-up and aftercare services as assigned.
- Provides life skills, coping skills, parenting skills, social skills and related education groups as assigned.
- Provides transportation to residents for referral appointments with various community agencies (medical, dental, psychological, etc.)
- Provides crisis intervention for clients, to include the use of approved verbal and physical intervention techniques.
- Assists in the implementation of programming in accordance with the Cal Farley’s Model of Leadership and Service.
- Utilizes curriculum approved by Cal Farley’s to promote goal achievement for family.
- Exhibits appropriate interaction with clients and families.
Requirements
- A Bachelor’s degree from an accredited university in a human services field is required.
- Master’s degree and related professional license is preferred. Related experience can be substituted.
- 2 years case management experience.
- A minimum of 24 months experience working with youth and/or families.
- Basic competency in Microsoft Office for Windows applications (word processing, database, spreadsheet, e-mail, presentation graphics, etc.) is required.
- Proficiency in general office equipment operation is required.
- Must possess exceptional time management and detail-orientation skills.
- Must be at least 21 years of age.
- Must possess a valid Texas driver’s license.
- Must be available for travel as needed.
