Position Summary:
The Home Health Coder is responsible for reviewing clinical documentation and assigning accurate diagnosis codes (ICD-10-CM) for home health patients in accordance with current regulations, coding guidelines, and agency policy. The coder plays a key role in ensuring documentation supports the services billed and optimizes reimbursement while maintaining compliance with federal and state regulations.
Key Responsibilities:
- Review and analyze clinical documentation from clinicians to assign accurate and appropriate ICD-10 codes for home health episodes.
- Apply official coding guidelines, CMS regulations, OASIS data, and agency-specific protocols to ensure accurate coding and compliance.
- Ensure all assigned codes reflect the patient’s current clinical status, comorbidities, and services provided.
- Collaborate with clinicians and case managers to clarify documentation and ensure accurate representation of patient conditions.
- Monitor changes in coding regulations and guidelines and adjust coding practices accordingly.
- Assist in preparing documentation for audits, reviews, or other regulatory needs.
- Communicate any discrepancies or concerns regarding documentation to the appropriate clinical staff.
- Maintain productivity standards and coding accuracy goals as defined by agency policy.
Qualifications
Education & Licensure:
- High school diploma or equivalent required
- Certification in medical coding (e.g., CPC, CCS, or HCS-D) preferred
- LPN or RN license (optional but may be preferred depending on the agency)
Experience:
- Minimum 1–2 years of coding experience in home health or post-acute care required
- Familiarity with OASIS documentation and Medicare regulations strongly preferred
Skills & Competencies:
- Proficient in ICD-10-CM coding
- Strong attention to detail and accuracy
- Excellent communication and critical thinking skills
- Ability to work independently and meet deadlines
- Proficiency with EMR systems (e.g., Homecare Homebase, WellSky, etc.)
Working Conditions:
- This is a non-clinical position and may be performed remotely depending on agency policy.
- Must have access to a secure internet connection and HIPAA-compliant workspace if working remotely.
