About The Role:

The Lead Family Housing Navigator works with families in the community who are experiencing homelessness or housing instability. The Lead will provide leadership to the team provide ongoing case management support to ensure program is meeting participants needs. The job involves; conducting intake screening and assessments of participants; and navigating participants through the process of referral to available housing resources. This role will also be responsible for helping program participants access related social services. The work will require occasional travel to apartments, cars, parks, bridge viaducts, forested areas, and other locations where people reside. This position serves on a team composed on EPHC and partner organization staff.

Principle Duties & Responsibilities:

• Lead a multi-agency mobile team providing; coordinated access assessment; resource navigation and referral; documentation collection; barrier mitigation; coordinated access queue & shelter waitlist management; and other flexible client assistance to support the rapid transition of people prioritized through coordinated access to available housing resources.
• Support the collaborative program that brings together a team of mobile housing navigators who engage in initiatives to expand access to the Homeless Family System of Care System and facilitate a smooth and rapid transition for households prioritized through this system to available housing resources (MHT, shelter, CoC programs). The team of housing navigators shall include Contractor staff, from El Programa Hispano Católico, SEI, PHFS, Our Just Future (HSI) and NAYA.
• Support households with providing referrals into other system and programs (i.e., adult, youth, survivors of Domestic Violence, Veteran, etc.), and to other appropriate services (mainstream benefits specialist, education, employment assistance and job training, food, legal, life skills training, outpatient health, mental health, substance abuse treatment, transportation, etc.).
• Work closely with households who are prioritized on queue and shelter waitlist to verify disabilities and length of time in homelessness, obtain other necessary documentation, and mitigate potential housing barriers to facilitate a rapid transition into housing upon referral. Upon referral, facilitate a smooth transition between the household and housing provider.
• Participate in frequent and regular Referrals Review Meetings and Leadership Team meetings.
• Monitoring of housing navigation tracking spreadsheets.
• Facilitate assignment and completion of VI-SPDATS in partnership with 211.Enter all necessary data into HMIS and ensure the team is entering data as well.
• Monitor, assist and train Navigation Team members on data collection and entry (entries and exits).
• Advanced knowledge of HMIS (ServicePoint) experience needed.
• Work closely with identified staff from the JOHS and to support the management of the HFSOC housing list and support process improvement related to Coordinated Access.
• Research and problem solving with collaborative partners.
• Record processes for Severe Weather Activation and communicate expectations to Navigation Team; set up shift scheduling for team during severe weather events; insuring staff coverage for the duration of the severe weather conditions.
• On-call coverage as needed when other Navigation Team members are unavailable for work.
• Generally, not responsible for carrying a caseload except in certain cases (culturally specific assessment needed, substituting for other team members).
• Facilitate referrals to the Shelter Waitlist; work with Shelter Waitlist Coordinator; gauge capacity for Shelter and Navigation team to accept new families onto their caseloads.
• Adherence to El Programa Hispano Católico policies and procedures
• Perform other duties as assigned.

Qualifications & Requirements:

• Must be bilingual (Spanish/English) and bi-culturally competent.
• Excellent communication skills, both verbally and in writing in Spanish and English.
• Bachelor's degree or equivalent experience preferred.
• Experience and knowledge working with a broad range of issues affecting homeless Latinx/ Hispanic and other BIPOC communities (Black, Indigenous, or People of Color).
• Intermediate proficiency in all programs of Microsoft Office; experience with data entry.
• Demonstrate judgment and discretion in dealing with confidential matters.
• Must be able to organize and prioritize work, be proactive, take initiative, follow through, and simultaneously manage multiple priorities to ensure goals are met in a timely manner.
• Willingness to learn new skills and take on new responsibilities.
• Strong analytical and strategic problem-solving skills.
• Ability to work well in team setting, as well as independently; be flexible and adapt well to different dynamics in a fast-paced work environment.
• Ability to support organizational and program specific mission and goals.
• Ability to lift 40 pounds.
• Access to a reliable motor vehicle. Must have driver's license, ability to obtain automobile insurance at levels required by agency (100/300/100).
• Satisfactory results from civil, criminal and motor vehicle background check required.

Compensation: This position starts at $26.25 per hour (DOE). Compensation is commensurate with skills and experience.

Location & Typical Working Conditions: This position may follow a hybrid work model based at EPHC's Gresham, OR office. Some remote work may be possible depending on role responsibilities and in coordination. Some in-person presence is important to foster team connection, support program delivery, and strengthen community relationships.Occasional travel and work outside standard business hours may be required based on project needs
