Make a Difference
The Director of Case Management will oversee Case Management for Community Hospital North and Community Heart and Vascular Hospital. The Director provides onsite leadership to assure that daily department operations function effectively and efficiently to meet the needs of all customer and regulatory groups. This position will provide consultation/direction to staff to facilitate compliance with departmental processes & policies as well as various regulatory agencies. This position provides direction, leadership and guidance to employees and customers and to facilitate continuous change and improvement. The Director will evaluate and monitor operational issues and make recommendations to promote cost-effective, quality care.
Exceptional Skills and Qualifications
Applicants for this position should demonstrate an ability to effect change by assuming leadership roles in interdisciplinary teams and support the growth and development of team members.
Graduate of an accredited School of Nursing and/or Social Work.
If a Registered Nurse, Bachelor’s Degree is required with a Master’s Degree required within five (5) years of
assuming the role.
If not a Registered Nurse, Masters of Social Work (MSW) is required.
Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC) or current LSW or LCSW from the State of Indiana for Social Work candidates.
Three (3) – Five (5) years of clinical experience in a hospital setting as a RN or MSW required.
Minimum of two (2) years of management/leadership experience required.
Two (2) – Three (3) years of case management experience required.
Demonstrate understanding of quality improvement processes at both the unit and network level.
Understands the relationship of individual patients, units, and/or departments to the network, to facilitate achievement of optimal goals within those relationships.
