Job Description
Job Title: Community Health Worker
Department: Community Health Action
Supervisor: Health Promotion Manager
Location: This position will be primarily located in La Casa; but it needs to be able to move between locations, as required. This position will work mostly on-site.
Classification: Non-Exempt/Hourly
Synopsis: The Community Health Worker position is instrumental to establishing La Clinica Del Pueblo’s Health Promotion strategies and interventions as well as supporting complementary programmatic activities, as needed, in the DMV area. This position will support a community-based approach to public health and the management of effective, culturally appropriate health promotion interventions; it will also be responsible for implementing community-based strategies and interventions aimed at reducing the incidence of chronic diseases and address barriers to healthy lifestyles among the Latino community.
Qualifications:
Required Education and Experience:
- High School/ GED Diploma, or an equivalent combination of education and experience from which comparable knowledge and skills may be obtained.
- Bicultural; Spanish speaker.
- One year of minimum experience working in a Latino and/or multi-ethnic communities.
- One year of minimum experience working in health promotion and/or with public health service delivery.
Preferred Education and Experience:
- Knowledge of Latino immigrant community in DC Metro Area, including Prince George County and Montgomery County in Maryland.
- Knowledge of support systems and resources available as well as understanding of the health needs of the community.
- One year minimum experience with supporting program implementation.
- One year minimum experience advocating for individuals and community needs.
- One year minimum experience facilitating small-group interventions.
- Experience in co-facilitating small-talk (‘charlas’) activities in community settings.
- Experience supporting community educational campaigns including but not limited to the design of educational material, delivering educational workshops, focus groups, and promotion through social media tools.
- Experience supporting the coordination of logistics for health promotion events.
- Experience uploading data into data collection systems.
Required Skills/Abilities/Certifications/Licenses:
- Excellent oral and written communication skills in both English and Spanish.
- Ability to work with all segments of the community; engaging.
- Strong ethical core.
- Ability to implement a health education social media strategy.
- Ability to encourage greater social media participation.
- Proficiency with Facebook, Instagram, YouTube, and other social media platforms.
- Computer literate in Microsoft Office (Word, Power Point, Outlook, Excel).
- Interpersonal and problem-solving skills, detail oriented, teamwork-oriented, self-starter, highly organized.
Preferred Skills/Abilities/Certifications/Licenses:
- DC, MD, or VA Community Health Worker Certification.
Duties and Responsibilities:
- Support implementation of program activities in the District of Columbia and serve as the leading Community Health Worker in the implementation of community-based health promotion interventions including:
- Facilitation of group/individual health education interventions.
- Build and maintain a strong professional relationship with a network of community service providers and stakeholders.
- Support integration of ‘promotores de salud’ in program implementation.
- Support tracking, ordering, and managing logistics support materials and incentives distributed in program activities.
- Track and upload activities participation data into the required reporting systems and maintain programmatic quality standards for all interventions provided.
- Collaborate with other LCDP programs and organizations in the development and implementation of larger community events.
- Support a navigation system to connect community members to existing services to address participants and patients’ social needs (food, housing, physical environment, transportation, legal services, and other).
- Implement health education strategies:
- Support in the creation/tailoring of culturally and linguistically appropriate health education/communication materials and social media strategies for virtual dissemination and in community activities, group interventions, and events.
- Support building individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, health education, social support, and advocacy.
- Other programmatic duties and responsibilities:
- Support in the recruitment, selection, and training of new peer health promoters.
- Assist in the implementation of other program activities and health promoters’ training as identified by the program manager.
- Assist in preparing programmatic reports by designated date, as required.
- Support program integration efforts with other community health workers within LCDP.
- Support supervisor in advocacy efforts with local and national coalitions, as appropriate.
- General duties and responsibilities:
- Represent LCDP with supervisor to the public, official organizations, and other groups by attending conferences, meetings, etc., as designated by supervisor and as related to the job position’s duties.
- Meet regularly with supervisor to report and review work plan.
- Submit expenses documentation and timecards into internal reporting systems, as necessary following internal guidelines.
- Attend required training opportunities to increase/maintain skills.
- Attend program, department, all staff meetings, and all mandatory La Clínica activities.
- Follow La Clínica’s Code of Conduct and all other personnel Policies and Procedures.
- Perform other duties, as assigned.
Supervisory Responsibilities:
- None.
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift up-to 30 pounds at times.
- Must be able to work outside and stand-up for prolonged period during outreach events.
- Must be able to work on a flexible schedule, available some evenings and weekends.
- Able to perform duties in different La Clinica’s locations as required.
- Able to perform duties and responsibilities remotely when needed.
