Base pay starting at $ 25.49- $26.58 per hour, based on Education and Certifications.

Social Support Coordinator provides our individuals services related to program planning, services, and supports with proficiency and person-centered value.
Required Qualifications
- Bachelor’s degree in human services related field.
- One year of experience in Human Services.
- Valid Driver’s license.
- Must be able to pass Criminal background and Drug testing.
Preferred Qualifications
- Bachelor’s degree in social work.
- Three years of experience working with individuals with intellectual and developmental disabilities.
Position Type/ Typical Hours
This is a full- time position(40 hrs/wk), days and hours of work are typically Monday through Friday, 8:00 a.m. to 4:30 p.m. Occasional flex hours, evening and/or weekend work may be required.
Essential Functions
- Ensure all Hazelwood residents are given ample and sufficient support and services necessary to ensure they can become an active member within the community, which will assist with enhancing all aspects of their lives.
- Support and advocate for residents through admissions, transitions, placements, team meetings, day to day life and special projects.
- Maintain personal contact with assigned residents and their families/guardians through providing counseling, arranging home visits, sending invitations to ILP meetings and conference calls, obtaining consents for treatment, and gaining approval of expenditures.
- Assists individuals to become gainfully employed off-campus. Identify community opportunities for individuals to participate in that complement their Enrichment Center classes or interests and will facilitate the development of friendships.
- Assists in transporting individuals to community outings, home visits, and transition visits.
- Assist the Transition Department in the entire transition process in accordance with the Kentucky Transition Policy. This includes coordinating and accompanying residents on pre-choice visits, responding to referrals, input on the development of the Transition Plan, assist in providing transition education to families/guardians, and assist in completing applications related to Medicaid SCL waiver funding, 202B Disability determination and community support services.
- Assist in providing resident staffing in the event of emergency staffing needs.
- Ensure compliance with all Intermediate Care Facility (ICF) regulations and all applicable state and federal laws and regulations.
