The Coordinator for Independent Living services is a dedicated advocate, passionately committed to empowering individuals with disabilities to live full, autonomous lives. Individuals receiving support are assisted in achieving independent living in their own homes. Responsible for developing and overseeing Individual Support Plans (ISPs), as well as evaluating plan outcomes, service quality, and stakeholder satisfaction. Ensures that all supports are aligned with organizational policies, funding requirements, and accreditation standards.

 

Essential Functions:

  • Develops and maintains individualized service plans (ISPs) for each client on the caseload, ensuring they foster opportunities for personal development. Monitors the implementation of these plans, evaluates outcomes, and oversees the quality of services delivered.
  • Oversees and maintains documentation for individuals on caseload. Supervises the accurate upkeep of individual records and submits reports as required. Ensures that all documentation complies with organizational, funding, and accreditation standards.
  • Conducts meetings with individuals on assigned caseload to review the support services provided, monitor progress toward plan goals, and gather feedback from those served.
  • Provides guidance to employees regarding ISP outcomes and objectives, and delivers training on any updates as necessary.
  • Attends meetings, in-services and opportunities for on-going education.
  • Provides coverage for direct support as needed.

Qualifications:

  • Bachelor’s degree in human services or related field, 1 year of experience working with individuals with disabilities and 1 year supervisory experience, or associate’s degree plus 3 years of experience working with individuals with disabilities
  • Must have knowledge of Microsoft Office suite including Outlook, Word and Excel.
