Summary: Provides culturally sensitive sexual health care and comprehensive medical services including pre-exposure prophylaxis, post exposure prophylaxis, and linkage to hepatitis C (HCV), STI, and HIV testing and treatment. Performs job specific duties in accordance with grant standards. Utilized Motivational Interviewing and a harm reduction approach. Demonstrates cultural competency in working with diverse populations using an anti-racist framework and equity lens.
Responsibilities:
- Identify barriers to seeking services such as transportation or language
- Provide access to free condoms and other safer sex supplies
- Conducts reminder calls for appointments
- Ensures procedures are in place to confirm services are received and needs addressed
- Assist with Data Entry and reporting
- Develop and maintain community resource binder for identified social determinants of health needs
- Participate in meetings with NYSDOH AI staff to review data, improve data utilization and assess progress in meeting contractual expectations
- Provides education regarding PrEP and screening for PrEP eligibility
- Provides follow up services that includes testing, medical care, and adherence support as per Al guidelines
- Provide education that includes the importance of screening, testing, adherence to care and medications, and methods to prevent STIs including condoms, PrEP, and PEP
- Assist with referrals to address barriers and support continued care
- Encourage increased acceptance of program by educating community members and CBOs
- Conduct community outreach engaging at risk patients in care and/or treatment
- Provide program training to Sun River Health providers and staff
- Supports Peer involvement in service delivery
- Perform HIV counseling and testing in the community
- Assist patients obtain medical, prevention and supportive services, including assistance with obtaining insurance, manufacturer (drug) assistance programs, and government medical assistance programs
- Implement interventions to address identified social needs include patient stabilization and/or crisis intervention when necessary
- Participate in quality improvement to evaluate and improve quality of services
- Assist with grant reports on program outcomes
