Summary of Program and Position: 

The Hana Like Home Visitor Program works with families to provide supportive coaching to teach strategies that enhance the parent or caregiver’s skills to provide a safe, healthy, nurturing environment for children ages birth to five years. Home visitors also work with prenatal mothers to support a wide range maternal-child health.

The Parent Educator provides evidenced-based visiting support services to prenatal mothers with young children to improve maternal and child health, child health and safety and child development knowledge. This position communicates with family members, internal team members, service providers and community partners and provides team leadership for service provision. This position is under the direct supervision of the Program Supervisor.

Essential Duties: 

  • Conduct evidenced-based home visiting services to families with young children, ages 0-5 in a manner consistent with standards and guidelines established by funders, state law and policies and procedures of the agency and in a manner that will improve the self-confidence of parents. In partnership with families, curriculum materials are used to promote positive parent and child interaction and parenting, increase child development knowledge, promote family's interaction with the community and improve health-seeking behaviors. Monitors the quality of services to all families, including the implementation of the team approach. 
  • In partnership with the family and the team, develops a meaningful Family Goal Plan (FGP) that addresses risk factors and maternal child health and safety. 
  • Administer developmental screens such as the ASQ and ASQ-SE, hearing and vision screen, parent assessments, such as AAPI/HOME, as well as those screens required by the MIECHV project such as screening for prenatal care, tobacco use, postpartum care, and breastfeeding. 
  • Enter and maintain all contact documentation on families by completing required forms, screens, and assessments, and maintaining and inputting documentation in program databases and funder database systems in an accurate and timely manner. 
  • Provide linkage to community resources and follow-up with support services as needed, teaching families to identify and access resources throughout the process. 
  • Conduct creative outreach activities designed to engage hard-to-reach families, in a consistent, yet respectful manner. 
  • Maintain effective working relationships with referral source, EID (Early Identification) community partners, state agencies, funders, and other appropriate providers of home visitation services.

Minimum Qualifications: 

  • High School Diploma or equivalent required 
  • Experience working with children and families required. e.g. pre-school/day care provider, parent educator, child skills trainer. 

Other:

  1. Possesses and maintains a valid driver’s license and daily access to an automobile.
  2. Possesses and maintains proof of no-fault insurance.
  3. Can obtain an acceptable pre-hire driver’s abstract.
  4. Can obtain an acceptable criminal background check upon hire, one year later, and annually thereafter. 
  5. Can obtain an acceptable CWS background check. upon hire, one year later, and annually thereafter. 
  6. Can obtain an acceptable National Sex Offender Registry Check upon hire, one year later, and annually thereafter.
