Description

General Description: Supports overall operations of the Hope Housing and Home First programs. Provides support to individuals who have experienced chronic homelessness with physical and/or psychiatric diagnoses, in permanent supportive housing programs, using a housing first, trauma-informed, harm- reduction model of service delivery.

Requirements

Essential Duties and Responsibilities:

  • Assists with problem-solving, activities of daily living (ADLs) and life skills tasks. Provides social support and a “listening ear” for client concerns.
  • Engaging clients to ensure the completion of their assigned household duties along with the maintenance of their bedrooms, supervises in household chore completion.
  • Assists in the preparation of and attends house meetings, group activities, and outings with clients. Builds life skill themes for meetings and prepares materials. While the case manager is meeting with individuals, Program Assistant assesses client needs for community resources (i.e., food and clothing banks, donated support, computer donations, follows-up with clients and their ongoing applications, concerns, maintenance and household needs, etc.)
  • Monitors safety of clients and communicates any concerns to the Assistant Program Director.
  • Drives clients (in own vehicle as well as Home First van for Home First clients only) to appointments and community resources as needed. When unavailable, assists team with arranging third-party transportation for the aforementioned appointments and needs.
  • Picks up and drops off supplies to clients including food, medication and other needed items.
  • Provides emergency intervention in response to crisis and follows protocol to address emergencies including contacting Assistant Director, Program Director or calling 911 when applicable.
  • Creates, updates, and distributes monthly calendar, newsletter for clients, as well as an ongoing list of community resources.
  • Participates in regular staff meetings. Must complete required training (i.e.; Mandt, CPR/Minor First Aid, Relias, Narcan, Montgomery County Continuum of Care, and any other mandatory trainings to keep certifications up-to-date).
  • Maintains inventory for household and office supplies and replenishes supplies as needed.
  • Keeps a log of client interactions in order to communicate needs and concerns with team members as appropriate.
  • Complete weekly tasks administered by the Assistant Program Director and Program Director. All administrative tasks will be done in the office (creating resource lists, calendars, newsletters, emails/notes to team members, etc.)
  • Able to lift and carry items of up to 10-15lbs.
  • Other related duties as assigned.


Required Knowledge, Skills, and Abilities:

  • High school graduate or GED required.
  • One year of related experience. Will consider combination of qualifications equal to educational and work experience.
  • Knowledge of Microsoft Office; including Outlook, Word, and Excel.
  • Ability to handle stressful situations and impromptu interactions with clients and staff due to urgent needs of the program.
  • Knowledge and experience in administering CPR and minor first aid preferred.
  • Good driving record and own a vehicle.
  • Ability to establish and maintain positive working relationships with people of diverse backgrounds and disabilities.
  • Individuals with lived experience of homelessness encouraged to apply.
