We are looking for a Home Health Nurse to join our small and specialized home care health team and provide exceptional care to our patients.

Position Highlights:
- Monday through Friday - flexible scheduling.

**Responsibilities: **
Administer skilled nursing care to patients requiring intermittent professional nursing services in accordance with department policies and procedures. Complete initial evaluation visit, regularly re-evaluate the patients nursing needs, initiates the plan of care and furnishes services. Coordinates services works with physicians and other personnel of changes in patients condition counsels patient and families. Participates in in-service programs, and supervises and teaches other nursing personnel.
Education: Must possess, as a minimum, a Nursing Degree and a current RN license from the state of Colorado.
Experience: Must have a minimum, one (1) year experience in a hospital, skilled nursing facility, or other comparable institution. Wound care experience a plus.
Licenses/Certification: Licensed RN in the State of Colorado. Valid Driver’s License, current automobile insurance, reliable transportation, and current CPR certification.
Compensation:
Compensation: $33.00 - $45.04/hour DOE In addition to the base salary, Nurses have the opportunity to increase their compensation through participation in our Clinical Ladder Program. This program recognizes and rewards engagement in organizational initiatives, professional development, leadership activities, and contributions to quality and patient safety efforts. Advancement on the Clinical Ladder is an opportunity to grow both professionally and financially.
