Pine Creek Care Center is seeking a dedicated and compassionate Licensed Physical Therapist to join our interdisciplinary care team. In this full-time role, you’ll help patients regain mobility, restore function, and enhance their quality of life through personalized therapy and evidence-based interventions. Key Responsibilities:
Patient Evaluation & Care Planning:
Conduct comprehensive physical therapy assessments.
Develop and implement individualized treatment plans in coordination with physician orders.
Collaborate with referring physicians and healthcare team members to ensure continuity of care.
Direct Treatment & Supervision:
Provide skilled therapy sessions tailored to patient needs and goals.
Supervise Physical Therapist Assistants, therapy aides, and students per state practice standards.
Assist in the training and guidance of Restorative Aides in collaboration with the nursing department.
Team Collaboration:
Participate in care planning meetings, utilization reviews, and interdisciplinary team conferences.
Effectively communicate patient progress, challenges, and discharge planning with the care team.
Documentation & Compliance:
Complete evaluations, daily notes, weekly progress reports, and recertifications using NetHealth documentation software.
Ensure all documentation meets state, federal, and third-party payer regulations.
Family Training & Discharge Support:
Educate patients and caregivers on home exercise programs and safety practices.
Recommend and help coordinate the provision of durable medical equipment (e.g., walkers, braces, grab bars).
Equipment Oversight:
Report equipment issues to ensure a safe and effective therapy environment.
Qualifications:
Education:
Bachelor’s degree in Physical Therapy required.
Master’s or Doctorate in Physical Therapy preferred.
Licensure:
Current Physical Therapist license in the state of California.
Must remain in good standing and meet continuing education requirements.
Skills:
Strong clinical, organizational, and communication skills.
Experience with Net Health (or similar EMR platforms) preferred.
Team-oriented mindset with a passion for quality care.
