Summary Description: Greater Mental Health of New York’s Care Management Program helps individuals access essential services, such as medical specialists, transitioning from hospital admissions, securing social supports, and housing. The Care Manager is responsible for delivering comprehensive care management services to individuals qualifying for the Health Homes Care Management program in Rockland County. This includes assessing, overseeing, and coordinating various needs such as medical, behavioral health, substance abuse, and social services. The role involves direct client care in the community or via telephonic services, participation in meetings, and adherence to agency policies while supporting recovery and utilizing evidence-based practices.

This is a Mandated Reporter position.
