Job Description

As a Registered Nurse for Outreach Health, you are responsible for performing on-site visits, assessing and developing a plan of care for all patients referred by OHC. You will ensure that patient service is delivered in compliance with the Texas Department of Aging and Disability Services (TDADS) and Health and Human Services Commission (HHSC) standards, licensure rules and regulations, and contract requirements.

RUG (MN/LOC) Certified or willing to take online courses to become RUG (MN/LOC) certified.

Essential Job Functions

• Completes patient medical assessments and reassessments as assigned, taking the necessary action to ensure our patient’s well-being.
• Provides direct skilled nursing to patients or make assignment of staff to provide skilled service.
• Assesses nursing tasks that will be delegated to an unlicensed OHC employee.
• Supervises delegated tasks according to the Board of Nursing Education (BNE) and policies of OHC.
• Communicates patient services needed to the assigned supervisor.
• Conducts coordination of care concerning patient issues according to licensure regulations and contract standards.
• Advises Field Supervisor of any changes in the patient’s schedule, caregiver, supervisory visits, or skilled nursing visits.
• Make assigned visits to each patient’s home in a timely manner.
• May substitute as needed and be available for Field Supervisors during absence as may be assigned by the Supervising Nurse and/or Administrator.
• May act as Alternate Supervising Nurse.
• Other duties as deemed necessary by the Pediatric Program Manager or Supervisor/ Manager

Required Skills, Education, Certification, License, and/or Experience

• Must maintain an active (unrestricted) RN license in the working state(s) with at least one (1) year of verified experience in home health or another nursing discipline
• Maintain a valid driver’s license and own a legally registered vehicle that holds the minimum liability insurance coverage
• A genuine interest and ability to work with people of all socio-economic groups
• Must be able to read, write, and comprehend English
• Must have one (1) year of experience in nursing in the health/social services industry
• Must have skills in problem-solving / interpersonal communication
• Must be detailed oriented and possess excellent time management skills

Preferred Skills, Education, Certification, License, and/or Experience

• Bilingual (Spanish and English)
• Experience in HHSC/DADS PHC/CAS and Managed Care programs

Pay Range: $22-$44/per visit + Mileage
