Street Outreach Manager (Program Leadership – Field-Based)
Salary Range: $80,000.00 – $90,000.00 annually, depending on experience, qualifications,
FLSA: Exempt
Location: Hybrid – Field Based (Regional / Multi-County)
Schedule: Monday – Friday | 8:30 AM – 5:00 PM (flexible for events and emergency response)
Work Structure: Hybrid with frequent field presence and regional oversight
The Street Outreach Manager leads field-based outreach operations with a dual focus on generating high-quality member referrals into CalAIM Enhanced Care Management (ECM), Community Supports (CS), and Behavioral Health (BH), while building durable, strategic relationships with community-based organizations (CBOs).
This role is accountable for outreach performance, referral pipeline development, and the establishment of trusted partnerships that drive sustained referral flow, coordinated care, and improved member outcomes.
