Nurse Case Manager - Clinic Setting

Location: Poplar, MT

We are seeking an experienced Nurse Case Manager for a 1-year contract with an ASAP start. This role focuses on coordinating patient care, managing case plans, and ensuring quality outcomes in a clinic environment.

  • Contract Length: 1 year
  • Start Date: ASAP
  • Pay: $55-$60/hr (DOE)

Responsibilities:

  • Assess, plan, and coordinate patient care and case management services
  • Collaborate with providers, patients, and interdisciplinary teams
  • Ensure compliance with clinic policies, regulations, and care standards
  • Manage documentation and reporting requirements

Requirements:

  • Active RN license
  • Must have Purchased Referred Care (PRC) experience
  • Clinic or outpatient experience preferred
