RESPONSIBILITIES

·        Licensed Practical Nurse Assessment Coordinator (LPNAC) has an important role in the case management and care plans of our residents

·        Oversee MDS scheduling and assessments

·        Initiate and create care plans

·        Case management oversight and responsibility

·        Maintain HIPPA compliance

REQUIREMENTS

·        Active and good-standing LPN license in the state of Pennsylvania

·        1 year of previous LPN experience

·        Long term care experience preferred

·        MDS experience/knowledge preferred
