OBJECTIVE/PURPOSE:
The Crisis Stabilization Support Worker (CSSW) provides a wide variety of direct client support services to persons admitted into the crisis stabilization facility. CSSW will also assist in basic medical care and be medication trained and then delegated to be able to help check-in admitted client’s medications and help administer those meds while the client is in the program.
MAJOR RESPONSIBILITIES:
1.0 DIRECT CLIENT SUPPORT SERVICES:
Administration of admissions and discharges once an individual has been accepted into Battelle House. This includes conducting the admission interview and checklist, making sure all forms and releases are signed as well as taking an inventory of personal belongings. While client is admitted the CSSW will provide support and answer questions helping the individual adjust to the house and the program. Assist in helping clients conduct outpatient intakes either in-person or electronically. Along with the Crisis Recovery Specialist, help form and facilitate client groups and morning/evening check-ins.
2.0 MEDICAL CARE AND MEDICATION ADMINISTRATION:
Provide basic medical care including checking vital signs upon admission/discharge and assisting the UCS nursing and medical staff with other appropriate duties. CSSW will be trained and then medication delegated and assist nursing and medical teams with obtaining and administering medications. This may include helping clients pick up medications.
3.0 FACILITY DUTIES:
Assist Crisis Recovery Specialist Assistant with cleaning and laundry when needed. Help with creating and preparing nutritious meal options for clients and assisting at mealtimes.
4.0 CLIENT IDENTIFICATION AND OUTREACH:
Provides outreach services to members of the community in need. Provides protection and advocacy of client rights, whenever possible. Provides persons served and families with referral information and assistance to meet needs not assured by our agency.
5.0 AGENCY PARTICIPATION:
Works to ensure the highest quality of services throughout the agency by participation in committees, quality assurance activities, and interdepartmental activities.
6.0 RECORD KEEPING:
Completes clinical and administrative forms in a timely manner for accurate billing, up-to-date clinical records, and appropriate information sharing. Completes all clinical documentation in a professional and timely manner including those required with admissions, discharges, and medication administration.
EDUCATIONAL REQUIREMENTS:
Bachelor’s degree in social work, psychology, rehabilitation counseling, or related field preferred. A minimum of one years’ experience working directly with individuals with mental health illnesses.
Eligible to obtain QMHP certification.
LICENSE REQUIREMENTS:
Valid Driver’s License
