Overview

We believe homelessness doesn't start with the absence of a home but rather the absence of HOPE. Through physical housing, our unique hope accelerator, and psychological wrap-around services, we give young people the support they need to not only find future housing and employment opportunities, but also their own personal purpose which will stay with them for the rest of their lives.

We believe that your trauma can transform your experience into divine purpose. We think of our house in Northridge LA as a little family (as this literally is our founder’s family home) and we call our youth members — similar to family “members.”

About Seed House Project

Seed House Project is a 501c3 that seeks to help young people see their full potential and remove systemic barriers of housing and resources in Los Angeles County by providing a reimagined and culturally centered approach to therapy, holistic engagement, and self-discovery,

78% of our former members have maintained housing and employment independently after our program.We currently partner with Village Family Services, which serves as the TAY Coordinated Entry System lead agency for the San Fernando Valley. We also work closely with Guardian Scholars (on-campus academic support programs for former foster youth in college), CSUN, Pierce College, Valley College, Mission College, West Valley Occupational Center, Safe Place for Youth, My Friend’s Place, iFoster, and others for student and TAY referrals.

Requirements

Clinical Responsibilities:

• Although this position is primarily virtual (approximately 90%), there are in-person documentation responsibilities required. This includes filing all assessments in the appropriate member files. The assessments we use include the PHQ-9, GAD-7, and ACES, which must be completed on a quarterly basis and properly documented. This role will manage a caseload of up to 35 members. Each member is required to complete a minimum of two sessions per month, or more depending on their individual treatment plan. Proper documentation and organization of all assessments and session records are essential to ensure compliance and continuity of care.
• Required team meeting Mondays at 11;30am ( Virtual)
• Once a month Purpose Therapy Meeting ( Virtual) time is flexible.
• Shall be responsible for developing and demonstrating appropriate skills in conducting psychological evaluations of patients, which may include a clinical diagnostic interview and /or psychological.
• Shall be responsible for developing and demonstrating appropriate skills in treatment and discharge planning.
• Shall be responsible for developing and demonstrating appropriate skills in determining diagnoses and treatment recommendations based on knowledge and identification of signs and symptoms of psychological disorders.
• Shall be responsible for developing and demonstrating appropriate skills in providing individual and group psychotherapy.
• Shall be responsible for developing and demonstrating appropriate skills in providing crisis intervention services or counseling, psychological emergency interventions and assessments, including determining need for 5150 evaluations.
• Shall be responsible for developing and demonstrating appropriate skills in providing consultation/liaison services to multidisciplinary program staff pertaining to patients’ diagnoses, mental status, interventions and treatment planning, as well as communicating changes in patients’ mental status or psychological disposition.
• Shall be responsible for developing and demonstrating appropriate skills and knowledge of current legal standards pertaining to professional practice, reporting requirements (i.e., suspected abuse; Tarasoff), professional boundaries and related issues.
• Provides timely, comprehensive progress note charting in patient’s Medical Record in accordance with JCAHO, State, County and Agency standards, to include individual sessions, group participation, crisis intervention, etc.

Administrative/Documentation

• Shall be responsible for maintaining proper documentation
• Shall be responsible for proper documentation of clinical information obtained through provision of clinical services rendered, and for obtaining signature of supervisors on all documentation.
• Shall demonstrate ability to conceptualize cases and prioritize treatment needs as evidenced by clinical documentation.

Qualifications & Skills

• EDUCATION
• Masters degree in Social Work or Master’s degree in Behavioral Sciences and registered as License Therapist
• EXPERIENCE
• Two years experience in providing substance abuse and/or mental health services, working with youth and/or families in a mental health setting, and prior experience with DMH documentation, although not mandatory, is preferred.
