The Care Team Lead oversees the operations of our care team, which includes all care team members. Reporting directly to the Care Team Director, this position is responsible for providing leadership, guidance, and support to the care team members, ensuring the accuracy of records, and keeping goals on track. This role encompasses training and development, quality assurance, and performance management to ensure the efficient delivery of high-quality care.

- Healthcare experience preferred.
- Minimum 1 years in a Care Team or Patient Navigation role is preferred.
- Minimum 1 years of Supervisory experience required.
