Attentive Care is a leading home healthcare agency with nearly 50 years of service in New York’s Capital Region. We are dedicated to providing compassionate in-home care to clients and supporting caregivers and home health aides. As an RN Supervisor, you will play a pivotal role in ensuring the highest standards of patient care and supporting our team of caregivers across Albany, Saratoga Springs, Schenectady, and Rensselaer.
Key responsibilities include:
- Conducting in-home client assessments and supervisory visits to ensure quality care delivery
- Developing and reviewing individualized care plans for clients
- Providing case management oversight and maintaining quality assurance standards
- Supervising and supporting home health aides and caregivers in their daily responsibilities
- Maintaining accurate documentation and ensuring compliance with regulatory requirements
- Collaborating with the office team to coordinate care and resolve client needs
- Participating in occasional on-call shifts, with additional compensation
Requirements
Required Qualifications:
- Current New York State Registered Nurse (RN) license
- At least 1 year of recent nursing experience
- English proficiency
- Authorized to work in the United States without visa sponsorship
- Ability to travel locally for in-home visits approximately 30% of the time
- Availability for occasional on-call shifts
Preferred Qualifications:
- Experience in homecare or home health
- Experience supervising or supporting home health aides or caregivers
- Experience using HHAeXchange
- Experience developing and reviewing patient care plans
- Experience with nursing documentation and regulatory compliance in a clinical or home health setting
- Experience conducting in-home patient/client assessments or supervisory visits
