Responsibilities

  • Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall function
  • Perform therapeutic exercises and interventions to help clients regain independence and reduce pain
  • Assess and document patient progress, modifying treatment plans as needed
  • Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes
  • Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention
  • Maintain accurate documentation and adhere to all legal, ethical, and professional standards

Requirements

What We’re Looking For:

  • Licensed Physical Therapist - you must hold a valid license in the state of Michigan.
  • Experience - while a minimum of one year in home health care is preferred, we’re open to fresh perspectives and new talent!
  • Team Player - you should have excellent communication skills and the ability to engage with patients and families effectively.
  • Compassionate Caregiver - a genuine desire to help others and promote their independence is vital.
  • Self-Starter - the ability to work autonomously, manage your own schedule, and track patient progress is key.
  • Transportation - a valid driver's license and reliable vehicle for travel to patient locations.
