Overview
We are seeking a compassionate and adaptable Direct Support Professional Floater to join our team. This vital role offers the opportunity to work across various settings, including group homes, and community-based programs. The ideal candidate will possess a strong background in patient care, or home health, with a focus on supporting individuals with diverse needs such as developmental disabilities, autism, dementia, and memory care. This position emphasizes direct support, care planning, and ensuring safety while fostering independence and quality of life.

Responsibilities:

 Works closely to teach and assist in self-care maintenance up to and including, cooking, household and program site skills, travel, social support, and personal hygiene assistance for individuals supported by People Arc ofSuffolk.

 Receive and remain up to date on all safe driving courses to safely assist in driving to and from activities,appointments or any needed transportation destinations.

 Provides assistance and oversight whole working on goals and accurately completes documentation and billing for personal goals developed by the individual supported via electronic health records (EHR).

 Plans and participates in recreational activities based upon the needs of the program site.

 Receive and remain up to date on all trainings to administer medication safely and in conformance with state regulations and specific training including maintenance of medical records (Residential Department only.)

 Other duties may include, but are not limited to, site maintenance, shopping, and oversight of the facility.

 May be required to assist in the lifting and/or transferring of individuals supported by People’s Arc of Suffolk.

 Receive and remain up to date on all HIPAA trainings while also maintaining confidential information regarding the person supported and their family.

 Received and remains up to date on training for safety and universal precautions.

 Exhibits care, consideration and professionalism while encouraging an independent and fulfilling lifestyle

$22.00 - $22.50 per hour

SATURDAY-WEDNESDAY FLEX HRS

WEDNESDAY-SUNDAY FLEX HRS

Locations:

Cluster 1, Commack, Dixhills, Huntington, Bayshore

Cluster 2, Westhampton , Southold, Greenport

Cluster 3, Bohemia, Coram, Medford, Old Field , Manorville

Requirements

 High school diploma or GED.

 Ability to work a rotating schedule

 Ability to work at multiple locations within an Assistant Directors cluster

 Dependable means of transportation to work at alternate houses as needed

 Ability to complete and pass Fleet Training, AMAP, SCIP and all additional medical or behavior training to support

 Valid NYSDL for 6+ months
