Patient Coordinator | Full-Time | Outpatient | Jacksonville, FL

At Great Strides Rehab, an H2 Health Company, we are committed to providing a welcoming, patient-centered environment where every individual receives high-quality care. As a Patient Coordinator, you will play an important role in supporting patients and families throughout their care experience while helping the clinic run smoothly.

Your Role:

  • Welcome patients and create a positive, friendly first impression
  • Schedule appointments and help manage daily clinic flow
  • Answer patient questions by phone and in person
  • Maintain accurate patient records and protect patient confidentiality
  • Communicate with healthcare providers to coordinate patient care
  • Assist with patient copays and balances
  • Obtain Plans of Care from referring physicians when needed

Requirements

  • High school diploma or equivalent
  • Previous healthcare or medical office experience preferred
  • Strong communication and customer service skills
  • Organized with strong attention to detail
  • Experience with electronic health records (EHR) is a plus
  • Ability to multitask in a fast-paced clinic environment
