We are currently seeking a dedicated and skilled Home Health Occupational Therapist to join our dynamic team. In this role, you will be responsible for assessing, planning, and implementing personalized therapeutic interventions to help clients achieve their maximum potential and improve their quality of life.

Responsibilities
- Conduct comprehensive assessments of clients' physical and functional abilities.
- Develop individualized treatment plans based on clients' needs and goals.
- Implement therapeutic interventions to enhance clients' daily living skills.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care.
- Monitor clients' progress and adjust treatment plans as needed.
- Educate clients and their families on rehabilitation techniques and assistive devices.
- Document patient progress and maintain accurate and timely records.
Requirements
- Valid Occupational Therapy license in the state of practice.
- Bachelor's, Master's, or Doctoral degree in Occupational Therapy from an accredited program.
- Previous experience in home health care or a similar environment preferred.
- Strong knowledge of occupational therapy practices and interventions.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate demeanor and commitment to patient-centered care.
