WHAT YOU’LL DO
• Provide skilled, one-on-one nursing care in the home
• Medication administration and management
• Monitor and document patient condition
• Perform treatments as ordered by the physician
• Educate and support family members
• Work closely with the Phoenix clinical team for ongoing support

WHAT YOU NEED
• Active Missouri RN or LPN license (or Compact License)
• Strong assessment and communication skills
• Compassionate, dependable, and self-motivated
• Valid driver’s license and reliable transportation
