We have expanded our offerings over the years to include Medicare Home Health, Rebound Therapy Services, Respiratory Care, and Hospice, all aimed at maximizing health, safety, and independence for our clients. Now, we are looking for a dedicated Per Diem Masters of Social Work (MSW) professional to join our dynamic team, focusing on Home Health and Hospice services. This position will play an integral role in ensuring our clients receive the highest level of care, support, and resources in their journey toward improved well-being. Responsibilities

  • Conduct comprehensive assessments of clients' psychosocial needs and develop individualized care plans.
  • Provide counseling and emotional support to clients and their families to promote coping mechanisms and enhance emotional well-being.
  • Collaborate with interdisciplinary teams to ensure holistic care and effective communication between healthcare providers and clients.
  • Educate clients and families on available resources, healthcare options, and strategies to navigate their care journey.
  • Facilitate family meetings and support groups to foster community engagement and support systems for clients.
  • Monitor and evaluate clients' progress and adjust care plans as necessary, documenting all changes in a timely manner.
  • Advocate for clients' needs and rights within the healthcare system, ensuring they receive appropriate services and benefits.

Requirements

  • Master of Social Work (MSW) degree from an accredited program.
  • Current state licensure or certification as a social worker (LMSW or LCSW).
  • Previous experience in healthcare settings, particularly in home health or hospice care, is highly preferred.
  • Strong interpersonal and communication skills to effectively interact with clients, families, and healthcare professionals.
  • Ability to conduct assessments, develop care plans, and document accurately and timely.
  • Compassionate, empathetic, and dedicated to improving clients' quality of life.
  • Proficient in managing time efficiently and handling multiple priorities in a fast-paced environment.
