Phoenix Home Care & Hospice is hiring a Home Health Admissions RN to complete Start of Care visits across Overland Park and surrounding communities. If you want meaningful one-on-one care without hospital chaos, this role was built for you.

What You’ll Do (and Why It Matters)

• Complete Start of Care admissions in patients’ homes
• Perform thorough assessments and build individualized care plans
• Educate patients and families on conditions like CHF, COPD, and Diabetes
• Manage medications and wound care
• Communicate changes clearly with physicians and the care team
• Work closely with PT, OT, and Speech to set patients up for success

This is nursing with time, autonomy, and purpose.

What We’re Looking For

• Active Kansas or Compact RN license
• Strong assessment and documentation skills
• Organized, compassionate, and confident in the field
• Valid driver’s license

Johnson, Wyandotte, Leavenworth, Miami, Franklin, Anderson, Linn, Atchison counties
