Occupational Therapist - Home Care needed in Philadelphia

KeystoneCare is seeking a dedicated and compassionate Occupational Therapist to join our Home Care team in the Philadelphia area. The Occupational Therapist will evaluate and assess patients’ needs, develop individualized treatment plans, and implement therapeutic interventions aimed at enhancing the patients' ability to perform daily activities. This position is crucial in promoting rehabilitation, improving functionality, and ensuring that our clients receive the highest standard of care.

Key Responsibilities:

  • Conduct patient evaluations and assessments upon admission to develop a tailored plan of care.
  • Collaborate with the patient’s physician and multidisciplinary team to outline achievable goals and necessary interventions.
  • Implement effective treatment strategies designed to improve strength, mobility, and daily living activities.
  • Monitor and document patient progress, adjusting treatment plans as needed, and communicating changes to the care team.
  • Provide education and support to patients and their families regarding treatment goals and home exercise programs.
  • Maintain accurate documentation following agency policies and ensuring compliance with regulatory requirements.
  • Demonstrate adherence to patient confidentiality and HIPAA regulations.
  • Participate in interdisciplinary team meetings and contribute to patient care conferences as needed.

Requirements

  • Master's degree in Occupational Therapy from an accredited program
  • Current Pennsylvania Occupational Therapy license
  • Minimum of one year of experience in a clinical setting, preferably in home care or hospice
  • Strong knowledge of therapeutic techniques and patient-centered care principles
  • Effective communication and interpersonal skills
  • Ability to work independently and manage a caseload
  • Valid Pennsylvania Driver's License and reliable transportation
  • Current professional liability insurance
  • Proof of COVID-19 vaccination or an approved medical or religious exemption
  • Proof of influenza vaccination or an approved medical or religious exemption
