Description

The Clinical Care Coordinator supports patient care by coordinating appointments, assisting with clinical tasks, and ensuring seamless communication throughout complex treatment plans. This hybrid role blends Certified Medical Assistant responsibilities with clinical scheduling and care coordination. The ideal candidate has strong experience working with oncology or other complex-care patient populations and thrives in a fast-paced specialty setting. 


 Key Responsibilities

  • Serve as a primary point of contact for patients, guiding them through their treatment plan, follow-up process, and next steps.
  • Schedule appointments, diagnostic testing, follow-up visits, and procedures across multiple providers.
  • Assist with clinical duties, including rooming patients, taking vitals, collecting medical histories, and preparing exam rooms.
  • Communicate provider instructions, test results, and care updates to patients with sensitivity and clarity.
  • Coordinate referrals, authorizations, and insurance requirements to ensure timely access to care.
  • Track patient progress, maintain follow-up schedules, and support continuity of care—particularly for complex or oncology-related cases.
  • Collaborate with physicians, clinical staff, and administrative teams to maintain efficient clinic operations.
  • Maintain accurate, timely patient documentation in the EMR.
  • Provide compassionate, patient-centered support to individuals and families navigating specialty care.

Requirements


  • Certified Medical Assistant required (CMA, RMA, CCMA, or equivalent).
  • Five (5) or more years of experience working in a complex care setting, preferably with oncology or cancer patients.
  • Prior Medical Assistant or clinical support experience required.
  • Scheduling or care coordination experience strongly preferred.
  • Strong interpersonal, communication, and organizational skills.
  • Experience with EMR systems.
  • Ability to multitask, prioritize, and work independently while supporting a team.
  • Compassionate, empathetic, and professional demeanor.
