Schedule: 36 hour per week, CNAs, day shift 8am-830pm, every other weekend
Experience: One to Three Years of training and/or experience required. One year of experience in a long
term care facility strongly preferred.
· Licensure:
o Valid Florida Board of Nursing certification (CNA License).
o Valid Florida State Driver’s License This is only required if colleague chooses driving herself/himself
as mode of transportation when reassigned to work at another facility. This is not a requirement if
the colleague chooses an alternative means of transportation when reassigned to another facility
based on business needs. Regardless of the mode chosen, both are at the colleague’s own
expense, with the exception of mileage reimbursement eligibility when traveling between facilities
while on company time.
· Continuing education: Meets State/Federal certification guidelines including 12 hours annual in-service.
· Certifications:
o Valid CPR certification from the American Red Cross or American Heart Association.
· Demonstrated experience in caring for the terminally ill and others with life threatening illnesses is
preferred.
· Typing/keyboarding skills using a tablet and/or computer strongly preferred.
· Must be able to remain flexible and work at alternate facility where there is a business need for
reassignment for one or more shifts at an alternative facility. When this occurs, the colleague is
responsible for their own transportation to the alternate facility and at their expense. If mode of
transportation is self-driving to that facility, the colleague must maintain an active, valid driver’s license and
proof of valid automobile insurance, both of which will be placed in the colleague’s HR file.
PHYSICAL REQUIRMENTS: · Ability to lift/pull/push up to 50 lbs. Also needs to have physical stamina to meet the demands of long shifts and physically demanding tasks. · Ability to ambulate throughout facility and stand for long periods of time. · Ability to bend, squat, kneel and reach. · Ability to make coordinated hand and finger movements, and possess strong coordination, and balance. · Visual acuity and ability to hear in order to auscultate patients, hear alarms, view monitors, etc. ORGANIZATIONAL SUPPORT: · Demonstrate professional accountability · Advocate for positive change · Display customer service excellence · Utilize all communication technologies and practices effectively · Participate in improving performance and quality · Manage resources effectively
JOB DUTIES/RESPONSIBILITIES:
1. Supports and carries out the organization’s mission, vision, and values on a consistent basis.
2. Performs only those personal care/home maintenance activities contained in a written assignment
(Hospice Aide Plan of Care) by a health professional employee.
3. Recognizes and reports unusual conditions, incidents or behavior of a patient and/or patient/family
interaction to the patient’s nurse or supervisory nurse.
4. Recognizes the role of the Hospice Aide and works within that scope of practice at all times.
5. Maintains utmost respect for confidentiality in care of the patient/family.
6. Actively participates in Interdisciplinary Team (IDT) meetings.
7. Demonstrates knowledge of Empath Health policies and procedures.
8. Utilizes proper body mechanics while lifting and moving patients, and during any other lifting activities.
9. Documents care given using appropriate electronic and paper methods to ensure all aspects of patient’s
care is recorded. Ensures plan of care is current and notifies nurse if any variation is noted between
patient condition and patient plan of care.
10. Participates in inservice and education programs provided by Empath Health.
11. Complies with all personnel policies.
12. Follows use of Standard Precautions in care of all patients. Adheres to all isolation precautions specifically
indicated for patients as prescribed
