Overview:

The Clinical Navigation Nurse Manager is responsible for the development, performance, and management of clinical patient care for the Clinical Navigation Team. The Nurse Manager will work as part of triad leadership team in making decisions that best support clinical staff and providers to ensure patients obtain access to care that is timely, effective, efficient, and safe. The Nurse Manager is responsible for oversight clinical navigation team productivity reports and applicable data to improve workflows that lead to improvement of the quality of patient care and enhance cost effectiveness. The Nurse Manager will also provide oversight and direction related to staff education, onboarding, and training of new hires. This position is considered Hybrid, which means that individuals in this position may work both at an approved offsite location and Onsite at a primary location or multiple locations based on Business Needs.

 

Responsibilities:

Essential Functions:

  • Collaborates with the patient’s care team including, primary care team, specialists, virtual care, home care, hospital team, and any others involved with the patient’s care to optimize clinical outcomes functioning from a call center and hybrid setting.
  • Participates as an effective member of the site Triad management team to ensure the clinical services team supports the overall operational and fiscal activities of the department to include staffing levels, budgets, financial and operating goals.
  • Manages the Clinical Navigation team. Establishes work assignment priorities, monitors the compliance with workflows, policies and procedures.
  • Responsible for interviewing/selecting candidates, approving payroll, time off requests, administering performance evaluations, training, and orientation of new hires, and for coaching and counseling staff.
  • Educates patient and family members on diagnosis, treatments, procedures, and medications in a call center setting.
  • Supports transitions of care activities with the patient’s medical home care management team or the Central Health case management team as indicated to support patients with complex health, social determinants of health (SDOH) and behavioral health needs, or frequent hospital utilization. Reviews clinical progress of patients and coordinates communications to referring, consulting, and primary care physicians.
  • Enhances the patient experience by supporting Provider teams with in-basket management, refilling medications, and triaging patients in a timely manner, scheduling of appointments, assist with the referral process following established protocols.
  • Participates in continuous quality improvement projects in order to better serve the patient, family and healthcare system to improve the quality of service provided. Serves on committees as assigned.
  • In collaboration with the Nurse Manager, maintains awareness of key performance indicators/metrics and coordinating interventions to prevent avoidable ER visits, hospital admissions and readmissions, etc.
  • Attends staff meetings and education offerings both in person and via teleconference/online as required.
  • Supports organizational initiatives to promote and maintain a strong positive workplace culture.
  • Performs other duties as assigned.

  • Strong application of Microsoft office and electronic health records.
Qualifications:

Minimum Education: 

  • Associate's Degree Graduation from an accredited School of Nursing (ADN)

Minimum Experience:

  • Two (2) years clinical supervisory experience in an ambulatory clinic and/or hospital setting 

Required Licenses and Certifications:

  • RN - Registered Nurse - State Licensure and/or Compact State Licensure Upon Hire
  • Basic Life Support through American Heart Association or American Red cross Upon Hire
