Title: Registered Dietitian
Department: Adult Day Services
FLSA Status: Non-Exempt
Reports To: Director
Summary: Responsible for conducting assessments, developing and implementing individual care plans, and monitoring Adult Day Services participants with nutritional needs.
Essential Functions:
⦁ Conducts initial and 6-month nutritional assessments for participants referred by the Registered Nurses or Social Workers.
⦁ Develops and implements care plans based on the individual’s nutritional needs, dietary history, food habits and preferences, and physician order.
⦁ Reviews the diet orders for participants on a quarterly basis and makes adjustments as needed.
⦁ Provides nutritional counseling on nutrition principles, dietary plans and food selection to participants and/or caregivers as needed.
⦁ Maintains appropriate records in participants’ charts.
⦁ Oversees the provision of food services in coordination with the Office Manager and Self-Help’s Nutrition Manager; advises on the preparation of therapeutic diets and evaluates food served at the center for conformance to quality standards and dietary prescription.
⦁ Provides in-service training for food service staff as needed.
⦁ Performs other duties as required.
Qualifications:
⦁ Currently registered by the American Dietetic Association.
⦁ Experience in working with the elderly or the chronically ill is strongly preferred.
⦁ Experience in working as a member of a multidisciplinary team is highly preferred.
⦁ Good organizational and communication skills.
⦁ Bilingual in English and Cantonese.
⦁ Must have TB clearance within 12 months prior to employment and maintain clearance annually thereafter.
⦁ Must acquire CPR and First Aid certificates within 6 months of employment and maintain current certificates thereafter.
