We have a rare opening for a Social Care Network Manager in our (hybrid) Tacoma office. As Social Care Network Manager, you will be responsible for the management and expansion of the Social Care Network of community-based social service organizations addressing community needs through the delivery of Health-Related Social Needs Services in Pierce County. This role works closely with the Community Care Hub Program and Operations team and expands the network through strategic identification and recruitment of social and Health-Related Social Needs Services providers into the network.


Here’s what you’ll be doing:

Social Care Network Management

  • Collaborate with leadership to create scalable network partnership strategies and plans.
  • Develop and implement a plan to expand the Social Care Network to meet network development goals.
  • Strategically identify, evaluate for readiness, and onboard new community-based organizations providing social and Health-Related Social Needs services in Pierce County, with additional focus on clinical or behavioral health providers.



Relationship Management and Workforce Development

  • Foster and maintain relationships with existing Social Care Network providers, addressing their needs and questions to ensure satisfaction and engagement.
  • Support Social Care Network provider readiness to join the Social Care Network.
  • Maintain quality of services and address partner-specific issue(s) to ensure network adequacy and capacity to accept Health-Related Social Needs services referrals.
  • Planning of community workforce trainings in partnership with internal and external sources.
  • Be primary point of contact for Elevate Health team and our Social Care Network provider partners.



Contract Management

  • Initiate and manage any Social Care Network’s agreements including renewal and development of new contracts to ensure compliance and communication of expectations.
  • Initiate contracting with potential care coordination agencies in partnership with the Community Care Hub and Finance teams.
  • Assist in hand-offs to Community Care Hub program and operations team for onboarding of community-based care coordination/case management service providers.
  • Collaborate with legal and compliance teams to negotiate and finalize provider contracts, ensuring fair terms.



Performance and Compliance Monitoring

  • Analyze provider performance metrics, identify areas for improvement and implement supportive action to partners as needed.
  • Ensure all Social Care Network providers adhere to regulatory guidelines and contractual requirements.
  • Conduct regular audits and assessments to support continuous quality improvement.
  • Provide Social Care Network provider partners with technical assistance and connecting partners to Community Care Hub and external supports as needed.



Data Management and Reporting

  • Maintain accurate provider records in designated database, ensuring all information is up-to-date and compliant with reporting and regulatory standards.
  • Work with Elevate Health’s analytics and Community Clinical Linkages and Community Resilience teams to evaluate potential and existing network partners and align with overall strategy.
  • Prepare and present reports on network performance, provider satisfaction, and recruitment efforts to management, leadership, and other community partners.


Here’s what we’re looking for:

  • Bachelor’s degree in Community Health, Public health, social services, non-profit management, social, or related field (Master’s degree preferred) AND
  • Minimum of 5 years of experience in program and operations management within community health or related sectors; OR
  • Any combination of education, professional, volunteer and lived experience that provides the necessary knowledge, skills, and abilities to perform the duties and responsibilities.
  • Experience working in communities addressing social and health care disparities.
  • Prior experience in customer/account management type roles ensuring customer satisfaction, adoption, and retention; specific experience in healthcare and provider relationship management.
  • Experience managing customer escalations, balancing customer expectations, and successful resolutions.
  • Motivated team member who identifies and addresses needs, collaborates to find solutions.
  • Prior experience working with diverse teams to influence organizational progress towards a goal.
  • Willing to travel, estimated at 25% of the time.
