We are growing! Greater Portland Health is expanding its nursing team to support and increase our capacity to assess, educate, and empower our patients to live healthy lives. Our nursing team is essential to our success in carrying out our mission to provide high quality patient-centered healthcare that is accessible, affordable, and culturally sensitive. The NCM will oversee clinical care to patients and serves in an expanded nursing role to collaborate with the Medical Team to provide a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services. The NCM will integrate evidence based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare.
Requirements:
- Registered Nurse with three to five years of recent experience required.
- Licensed at an RN level by the State of Maine Board of Nursing.
- Bachelor’s degree in nursing preferred.
- BLS Certification.
- Experience in care management/care coordination to include care planning, resource management, and utilization management desired.
