Completes face to face assessments and healthcare services under the general supervision of the agency’s Medical Director and hospice physicians, for agency hospice patients.

Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

ESSENTIAL FUNCTIONS:

  • Evaluates physical/psychosocial health status through performing comprehensive health assessments as required by agency policies.
  • Accountable for completion of face to face assessments within required timeframes.
  • Collaborates with physician, patient/family and interdisciplinary team regarding assessment findings, care needs, and revisions to plan of care.
  • Establishes and maintains positive working relationships with patients, family members, co-workers, physicians and referral sources.
  • Performs timely and accurate documentation in accordance with agency policy.
  • Abides by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).

MINIMUM KNOWLEDGE AND SKILLS REQUIRED:

  • Knowledge of MS Office software.
  • Keyboarding skills.
  • Demonstrated ability to communicate effectively.
  • Multitasking and prioritization skills.
  • Ability to apply advanced nursing principles.

PREFERRED KNOWLEDGE AND SKILLS REQUIRED:

  • Hospice and/or Oncology experience.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:

  • MSN.
  • Advance Practice Degree.
  • Certification in Family Practice or CHPN within three months of hire.

PREFERRED EDUCATION AND EXPERIENCE:

  • Certification in Family Practice
  • CHPN

REQUIRED LICENSE/REGISTRATION/CERTIFICATION:

  • Current licensure as an APRN in the State of Kansas.
  • NPI Number and DEA Number
  • Kansas State Driver’s License
  • Auto Insurance equal to or greater than the Kansas insurance requirements.
  • BLS – BLS is required within three months of hire
