ESSENTIAL FUNCTIONS:
- Evaluates physical/psychosocial health status through performing comprehensive health assessments as required by agency policies.
- Accountable for completion of face to face assessments within required timeframes.
- Collaborates with physician, patient/family and interdisciplinary team regarding assessment findings, care needs, and revisions to plan of care.
- Establishes and maintains positive working relationships with patients, family members, co-workers, physicians and referral sources.
- Performs timely and accurate documentation in accordance with agency policy.
- Abides by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).
MINIMUM KNOWLEDGE AND SKILLS REQUIRED:
- Knowledge of MS Office software.
- Keyboarding skills.
- Demonstrated ability to communicate effectively.
- Multitasking and prioritization skills.
- Ability to apply advanced nursing principles.
PREFERRED KNOWLEDGE AND SKILLS REQUIRED:
- Hospice and/or Oncology experience.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:
- MSN.
- Advance Practice Degree.
- Certification in Family Practice or CHPN within three months of hire.
PREFERRED EDUCATION AND EXPERIENCE:
- Certification in Family Practice
- CHPN
REQUIRED LICENSE/REGISTRATION/CERTIFICATION:
- Current licensure as an APRN in the State of Kansas.
- NPI Number and DEA Number
- Kansas State Driver’s License
- Auto Insurance equal to or greater than the Kansas insurance requirements.
- BLS – BLS is required within three months of hire
