Foxdale Village, a life plan community guided by Quaker values, is a non-profit Continuing Care Retirement Community (CCRC) located in State College, PA. Foxdale was founded in 1985 by members of the State College Friends Meeting and is founded on the values of treating all with dignity and respect. Foxdale is regarded as a premier healthcare facility both locally and nationally and has been voted "Best Retirement Community in State College, PA" for over a decade. Foxdale Village includes 57 apartments and 148 cottages and offers a range of health services with 46 beds in skilled nursing for short and long-term care, two memory care neighborhoods, and 55 beds in personal care, all with private accommodations. We currently have an opportunity for a new Health Information Coordinator to join our team.


We are hiring for Full-Time position and expected work 37.5 hours per week (8:30am-4:30pm) Monday-Friday.


The Health Information Coordinator at Foxdale Village develops and manages services needed to maintain clinical records, reports, and required medical record logs. Oversees and maintains discharge, admissions and transfer tracking for clinical record requirements. Participates in committee meetings, maintains confidentiality, and manages facility policy.


The Health Information Coordinator Job Duties Include:

  • Ensure that resident clinical records are maintained in accordance with professional practices and regulatory standards. Assure that each record is kept confidential, complete, accurate, and readily accessible.
  • Accurately enter reports and maintain the flow of reports to the clinical record in a timely manner.
  • Maintain thinned and discharged resident clinical records.
  • Oversee and maintain record retention schedule and arrange for record destruction.
  • Assure that clinical records are stored in such a manner to prevent loss, destruction, or unauthorized usage.
  • Responsible for ordering, receipt, and distribution of forms and equipment used for the clinical record.
  • Code and index diagnoses upon admission to Foxdale Village.
  • Participate in regulatory triple check.
  • Provide in-services and education for department on health information issues or updated regulations.
  • Obtain complete and accurate records within established time frames.
  • Ensure that all required information and copies of reports have been received from the transferring unit or facility and follow-up as appropriate.
  • Maintain Admission and Discharge Logs.
  • Collect and correlate data from the clinical records as requested.
  • Completes monthly reports for both Personal Care and Skilled Nursing.
  • Understand all aspects of the clinical computer system. Assist and maintain electronic health records.
  • Support and assist in carrying out corporate compliance initiatives as assigned.
  • Oversee and assist with medical coding via ICD 10 for all medical records.
  • Facilitate authorization process for medical needs and justification
  • Maintain current policies, and procedures for the health information department.
  • Participate in meetings and committees such as daily morning meeting, QAPI, and Medicare documentation review, etc.
  • Conduct and facilitate focused audits on identified area for improvement to assure accuracy, quality and compliance.
  • Facilitate all medical record requests for resident insurances
  • Obtain prior authorizations needed for resident insurance and orders
  • Oversee physician required visits for both internal and external contractors
  • Audit clinical records according to established criteria to assure completeness, accuracy, internal consistency, and regulatory compliance, including admissions and discharges.
  • Maintain and control the release of information to authorized persons.
  • Maintain morning meeting reports and send out daily to interdisciplinary team.
  • Type reports or correspondence as needed.
  • Maintain and update clinical record policies and procedures.
  • Manage the credentialing process for physicians and other providers per facility policy.
  • Maintain current signature lists of staff for medical records.
  • Promptly assist physician and nursing personnel in locating clinical record information as needed.

Minimum Qualifications:

Education: Successful completion of a formalized Health Information program of study with receipt of Associate’s Degree and Registered Health Information Technician certification. Recent graduates of a Health Information program are eligible for employment prior to taking certification exam, with continued employment contingent on successfully passing exam.

Experience: One year of experience in health care field, two or more years preferred; knowledge and understanding of regulations, survey process, accreditation standards, professional standards of practice related to long term care, supervisory and management skills preferred.

Skills: Working knowledge of medical terminology, Medicare eligibility and reimbursement methods; computer skills, Understands coding and ICD-10-CM coding appropriate for long term care, excellent interpersonal and communication skills; ability to cultivate relationships and work within a team environment; flexibility to occasionally work nights and weekends, as necessary;

Language: Must be able to effectively communicate in English.

Math: Must be able to complete tasks using basic mathematical skills.
