Title: Case Manager
Description: This role within the Medical Case Management team at Central Outreach Wellness Center requires an individual to be patient-centered and culturally competent while working with a diverse patient population within our LGBTQ+ health center. Their primary role is to navigate clients through the processes that connect them to the medical benefits, rehab services and housing services available to them.
Services we Provide:
- Resources for connecting patients to insurance including MAWD, SBPB, Compass, Gilead, SNAP, Medicaid, etc.
- Housing resources for people with a steady income.
- Connecting patients to Rehab if they are ready to go to ASAP. If not, we connect them to Resolve
- Connect patients to outside resources for emergency housing, resume building, career link for employment, financial empowerment
General Duties and Responsibilities:
- Document patient encounters in our EMR System
- Stay knowledgeable of community resources offered to the population we serve
- Work collaboratively with clinic staff to address any concerns of the patient
- Answer and respond to tiger text and e-mails in real time
- Navigate clients through the processes needed to connect them to the programs and services available
- Be available to the clients throughout the medical case management processes
- Proactively manage your caseload of patients and participate in weekly staff meetings to discuss any issues
Specific Duties and Responsibilities
- Be the point person to coordinate patient intakes at the clinic
- Help manage the caseload on the spreadsheet as the CM team needs
- Be patient and attentive to patients’ needs
- Quick to adapt to high pressure situations
- Always find a solution to the patients’ needs, and assist them in completing tasks
- Keep an organized work ethic and document all encounters
Qualifications
Requirements:
- Bachelor’s degree or equivalent customer service skills preferred
- Strong communication and people skills
- Experience with patient-centered care
- Ability to manage difficult situations utilizing de-escalation techniques and conflict resolution skills
- Ability to work collaboratively in team-based care.
- LGBTQIA cultural competency
- Understanding of the relationship between physical health, mental health, and social determinants of health
- Commitment to providing high quality care to historically underserved populations
- Knowledge and comfort working with patients with drug and alcohol use disorders
- Ability to adapt to a work environment that is constantly changing and not always structured
- Completely understand the healthcare privacy laws as outlined in HIPAA
- Attend morning huddles
- Ability to multi-task
- Maintain a professional attitude and demeanor while working in the clinic
- Separate personal life from work
