Position Summary 

The Cardiologist will deliver expert cardiovascular care within MCHC’s outpatient setting, diagnosing, treating, and managing patients with a wide spectrum of heart and vascular conditions. This provider will partner with the multidisciplinary MCHC health team to ensure care is coordinated, holistic, and aligned with our FQHC mission and regulatory frameworks (including HRSA/FTCA compliance). 

Essential Duties and Responsibilities 

  • Provide high-quality cardiac care consistent with current clinical guidelines and evidence-based practice. 

  • Perform and interpret diagnostic studies (e.g., EKGs, echocardiograms, stress tests, and Holter monitoring). 

  • Evaluate, diagnose, and manage both acute and chronic cardiovascular diseases—including hypertension, coronary artery disease, arrhythmias, heart failure, and more. 

  • Develop individualized treatment plans for complex cardiovascular patients and monitor outcomes. 

  • Collaborate with primary care, specialty teams, and care coordination staff to ensure seamless patient management. 

  • Participate in case conferences, quality-assurance initiatives, interdisciplinary meetings, and other programs to improve patient outcomes. 

  • Serve as a consultative resource within MCHC for cardiovascular care and preventive strategies. 

  • Ensure prompt review and follow-up of diagnostic results, abnormal findings, and transitions of care. 

  • Maintain complete, accurate, and timely documentation within the electronic medical record (EMR), consistent with organizational and regulatory standards. 

  • Promote culturally competent communication and care delivery reflective of MCHC’s diverse patient population. 

  • Engage in community education and outreach efforts to support cardiovascular health and disease prevention. 

  • Maintain all required licensure, board certifications, and continuing medical education (CME) obligations in good standing. 

  • Perform other duties as assigned by the Chief Medical Officer and/or Chief Operating Officer. 

 

Qualifications and Requirements 

  • Medical degree (MD or DO) from an accredited medical school. 

  • Valid and unrestricted physician license in the State of New York . 

  • Board-Certified or Board-Eligible in Cardiology. 

  • Preferably at least two (2) years of post-residency clinical experience—ideally in a community health or FQHC setting. 

  • Current BLS and ACLS certifications. 

  • Demonstrated ability to provide compassionate, culturally sensitive care to underserved populations. 

  • Excellent communication, diagnostic, and interdisciplinary collaboration skills. 

  • Must perform all job responsibilities in full compliance with applicable laws, rules, regulations, and organizational policies.
