Licensed Physical Therapy Assistant (PTA) | Care Resources, GR na Care Resources

• Provide physical therapy services under the supervision of licensed therapists, including establishing restorative and home exercise programs.
• Prepare clinical documentation and progress notes in compliance with agency policies and HIPAA standards.
• Collaborate with the Therapy Department and interdisciplinary team to ensure high-quality client care and effective discharge planning.
• Educate clients, families, and staff on therapy procedures to support ongoing care and rehabilitation.
• Maintain and manage therapy equipment, assist in program development, and participate in in-service training and departmental goal setting.
• Travel to participant homes or facilities as needed, requiring a valid driver’s license and reliable transportation.

• Some travel to the PACE of Lowell clinic as needed, and occasionally travel to client homes in Kent, Ionia, Barry, Allegan, and Ottawa County

• Requires licensure as a PTA  
• Minimum of 1 year of PTA experience preferred
• Top candidates will have prior experience working with a frail or elderly population
• The ability to multi-task and to work independently as well as within a team setting
• A passion for those whom we serve through this amazing program

• Wages start at $23.67 without experience.
• Wages are based on years of experience and will vary depending on that factor.

ABOUT CARE RESOURCES:

Care Resources operates the PACE program (Program of All-inclusive Care for the Elderly) which is located in at 4150 Kalamazoo Avenue SE in Grand Rapids.  Care Resources provides and coordinates all medical care and social services to eligible participants. We provide all physician and nursing care, prescription drugs, durable medical equipment, dental, eye and foot care, hospital care, medical transportation, home care, emergency, rehab, social and recreational activities at the Day Center. To be eligible for the PACE program, seniors must be 55 and older, a Kent County resident, able to live safely at home and meet the nursing facility level of care.
