Position Summary
We are seeking a dedicated and compassionate Community-Based Psychiatry provider, APRN, DO or MD to join our multidisciplinary behavioral health team in Madison, WI, serving both the Community Intervention Team(CIT) and Enso Residences – Supportive Services. This position focuses on providing psychiatric care and consultation to individuals with serious mental illness (SMI), co-occurring disorders, and underserved populations in community-based settings.
You will work collaboratively with case managers, social workers, therapists, nurses, and peer specialists to deliver holistic, person-centered care that meets people where they are — in clinics, shelters, supportive housing, or in their homes.

Key Responsibilities:
- Conduct psychiatric evaluations and assessments of individuals in community settings.
- Diagnose mental health conditions and develop individualized treatment plans.
- Prescribe and manage psychiatric medications in alignment with evidence-based practices.
- Provide psychiatric consultation to interdisciplinary care teams.
- Participate in case reviews and treatment planning meetings with staff.
- Support integrated care by collaborating with primary care providers and community partners.
- Maintain accurate and timely clinical documentation in the electronic health record (EHR).
- Engage in outreach to individuals who may be disengaged from services.
- Participate in quality improvement initiatives and team trainings.
- Provide supervision and guidance to residents, medical students, or advanced practice providers as appropriate.
Qualifications
Qualifications
- Current Wisconsin licensure as an Advanced Practice Nurse Prescriber (APNP).
- Master’s or Doctorate degree in Nursing from an accredited program.
- Board certification in Psychiatry/Mental Health (PMHNP-BC) preferred.
- Demonstrated experience in psychiatric/mental health practice.
- Ability to provide both independent psychiatric care and collaborative team-based support.
- Excellent communication, collaboration, and clinical decision-making skills.
- Strong organizational, clinical, and communication skills.
- Commitment to Tellurian’s mission and values of compassion, dignity, and recovery-focused care.
- Valid driver’s license, reliable vehicle, and proof of insurance (community/home visits may be required).
Preferred
- Experience in Assertive Community Treatment (ACT), community outreach, or integrated behavioral health models
- Bilingual or multilingual abilities (Spanish, Hmong, or other languages common in the region)
- Knowledge of trauma-informed care and harm reduction approaches
Work Environment & Schedule
- Clinic setting for primarily community-based individual with potential travel to homes, shelters, and partner agencies within Madison
- Some telepsychiatry options available
- Monday–Friday, 8:00am–5:00pm
- On-call responsibilities for 1 week term every 5-6 weeks
