Job Purpose & Position Overview:
The Case Manager will work exclusively with the Wellness Advocacy Zones: Opportunities for Kinship Involvement (WAZOKI) project. The WAZOKI project will work with a Winnebago community coalition to increase kinship involvement in pregnancy with the Winnebago community.
The Case Manager plays a vital role in supporting birthing families through culturally respectful, trauma-informed care coordination. This position bridges traditional Indigenous knowledge and modern maternal health systems, ensuring that families receive holistic support from pregnancy through postpartum. The Case Manager works alongside doulas, Tribal health providers, and community partners to uplift birth equity and strengthen intergenerational wellness.
Specific Job Duties/Responsibilities:

Work closely with WAZOKI project team and Community Coalition to develop plans that promote the social, physical, and intellectual growth of all mothers and her support system with Ho-Chunk values as center of daily activities.

Develop communication plans for outreach to mothers and their support people

Contributes to ongoing program development and improvement.

Provide monthly report to program director.

Conduct culturally sensitive intake assessments and develop individualized care plans

Identify and address barriers to care (transportation, housing, food security, mental health)
-
Provide emotional support rooted in Indigenous values and healing practices
-
Collaborate with doulas to incorporate Tribal birthing traditions, language, and ceremony

Advocate for culturally safe care within hospitals, clinics, and social service agencies
-
Facilitate connections to elders, traditional healers, and community knowledge keepers
-
Schedule and track doula visits, prenatal/postpartum appointments, and referrals
-
Maintain accurate, confidential records in alignment with Tribal and HIPAA standards
-
Assist with data collection for grant reporting and program evaluation
-
Build relationships with Tribal departments and local service providers
-
Organize community education sessions (e.g., childbirth, parenting, wellness)
-
Participate in Tribal health events, talking circles, and interagency meetings
Qualifications
Qualifications/Skills:
-
Knowledge of Ho-Chunk language and culture
-
AA Degree or equivalent experience in Social Work, Public Health, Human Services, or Indigenous Studies preferred; minimum high school diploma or verifiable GED certificate. Preference will be given to those with an AA or Higher Degree.

Must have a valid driver’s license and must be able to meet driver’s eligibility guidelines set forth by the Winnebago Tribe of Nebraska’s insurance carrier to become an insured driver.

Deep understanding of Indigenous lifeways, historical trauma, and reproductive justice

Experience working with Native families, especially in maternal or child health
-
Strong communication, organization, and crisis intervention skills
-
Familiarity with Tribal health systems and local resources

Experience with trauma-informed care and culturally responsive case management
-
Ability to navigate both Western and Indigenous health systems with respect and humility
-
Commitment to uplifting Indigenous sovereignty, wellness, and family strength

Must have advanced computer skills. a. Proficiency in Microsoft Office, Word, Excel, and Publisher.

Must possess grammatical competence.
