Veterans Directed Care Person Centered Counselor na DIRECTION HOME OF EASTERN OHIO

Description

Under the general supervision of the Director of Home and Community Based Services and the direct supervision of the Veterans Directed Care (VDC) Supervisor, the Person-Centered Counselor (PCC) is responsible for knowing and abiding by Veterans Directed Care program contractual expectations. The PCC is expected to support the Veteran in self-directing and coordinating services that have been approved by the Department of Veterans Affairs (VA).

1. Ability to effectively communicate with the general public, in person, by phone, and via email,
2. Ability to comprehend written material related to essential job functions,
3. Ability to accurately document facts related to essential job functions,
4. Ability to comprehend, recall, and apply facts related to essential job functions,
5. Ability to analyze, evaluate, and implement a reasonable course of action based on available information,
6. Ability to lift/carry 20 lbs. upstairs,
7. Ability to travel to community and individual locations within our four-county service area (Ashtabula, Mahoning, Trumbull and Columbiana counties),
8. Ability to occasionally travel outside of our four-county service area including but not limited to Columbus, Ohio and
9. Ability to ambulate enough to access private homes/locations which may not be readily accessible.

Qualifications

• College Degree preferred with at least one year’s prior experience in home health care, medical social work and/or geriatrics. 
• Demonstrates working knowledge and skills necessary to provide the assessment and care management required by the Veteran.
• Applicant must demonstrate flexibility in schedule and in adapting to rule changes and productivity changes.

Applicant must possess a valid driver’s license and proof of automobile insurance. 

Must also pass a criminal background check and drug screen.
